In order to register your child, you must bring the following documents to the Registration Center:

• CHILD'S BIRTH CERTIFICATE OR PASSPORT
• CONNECTICUT STATE HEALTH ASSESSMENT (BLUE FORM)
• MANDATORY - PROOF OF RESIDENCY - Must provide documents from any two of the numbers (1,2,3,4,5) listed below.

1. Copy of one of the following at address within district in parent's name:
   1. deed to home
   2. escrow papers or signed mortgage commitment
   3. rent receipt for latest month with landlord's name, address, and telephone number
   4. dated rental agreement showing student(s)' name(s) or signed and dated letter from landlord or homeowner acknowledging parent's residence and student(s)' residence
2. Either of the following showing address within district in parent's name:
   1. most recent utility bills (phone, gas, electric)
   2. deposit receipt for gas, electric and phone service start-up
3. State ID card with picture or driver's license showing current address (no temporaries). If address has been changed, card or license will not be accepted.
4. Either of the following showing delivery to residents' address within district in parent's name:
   1. major moving company receipt for moving household goods
   2. receipt from local firm showing delivery of newly purchased major appliance or furniture
5. Current active bank account checkbook with name and address imprinted. (Bank may be contacted to verify existence of account).
