PERSONNEL 4114 EMPLOYEE MEDICAL HISTORY AND PRE-EMPLOYMENT PHYSICAL EXAMINATION RECORD Each applicant for regular employment by the Board of Education shall pass a pre-employment physical before any such applicant shall be officially hired. The judgment whether any applicant has passed his/her physical examination shall be made by the administration with the advice of the Town Health Director. The administration, with the advice of the Town Health Director, will provide a proper form for recording the necessary health data. This form shall become a part of the employee's permanent personnel record. The prospective employee is responsible for the cost of such a physical and may use any licensed physician.
